Pay Range: $33 - $59/hour. Employee benefits include, but are not limited to, health insurance (medical, dental, vision), 401(k) plan, and paid sick leave (depending on work location).

Key Responsibilities:

• Coordinate and manage the patient’s care journey from admission through discharge.
• Collaborate with physicians, interdisciplinary teams, and families to ensure effective transitions of care.
• Assess, plan, implement, and evaluate individualized care plans.
• Facilitate communication between patients, care teams, and external providers to achieve positive outcomes.
• Provide patient education and support to promote self-management and adherence to treatment plans.
• Document and track patient progress in accordance with hospital and regulatory standards.

Key Requirements and Technology Experience:

• Key Skills; BSN, BLS, experience as a Registered Nurse .
• Education: Graduate of an accredited School of Nursing; BSN preferred.
• Experience: Minimum 2 years of acute care nursing experience required. Previous care coordination or case management experience preferred.
• Licensure: Active Registered Nurse (RN) license in the state of Georgia.
• Certifications: BLS certification required; ACM or CCM certification a plus.
